IMPORTANT: Once you submit your enrollment, your membership will be applied to your patient profile. A valid credit card is required to be active and on file with us to maintain your membership. Membership is renewed automatically on the anniversary date each month unless a request to cancel is received by our office. Cancellation requests received within ten days of renewal will be effective after the next billing cycle.
